This training has been developed for care workers in the community and aged care settings that would like a better understanding of chronic conditions and how to better support their clients

Suitable for support workers, health care employees, assistants in nursing and nurses working in the Community or Aged Care setting.

This 1 day workshop includes a workbook and covers:

  • Chronic conditions in the community and aged care setting: common chronic conditions, statistics,
  • Risk factors, community and aged care teams and case studies
  • A closer look at these chronic conditions:
  • Diabetes
  • Cardiovascular Disease
  • Chronic Kidney Disease
  • Respiratory Disease
  • Musculoskeletal conditions
  • Cancer
  • Social and Emotional well being
  • Dementia

Presented by – Jane Calligeros (CDM Plus)

Jane has 18 years of Nursing experience; the last seven years working in general practice. Jane began her career in 2000 after completing her Bachelor of Nursing in Newcastle. Jane followed her passion of Primary Health Care in 2006 including Post Graduate qualifications in Chronic Disease Management in 2015. Jane was nominated in 2015 and 2017 for an APNA Best Practice Award in Chronic Disease Management for her accomplishments in Indigenous Health.
